I picked up this fascinating machine from eBay and found that it is made by The Mechanical Orguinette Co., New York. I'm going to do some research, but I envision making a wonky mini MIDI thing, or maybe a pipe tester.
Here's what's inside this box. There are three bellows that are pumped by a crankshaft and a fourth bellows that appears to be a reservoir. It looks like these bellows are designed for suction rather than positive pressure.
I found this label inside the box. It says,
